Specifications
Uses & Indications
Follitropin alfa is a recombinant human follicle-stimulating hormone (r-hFSH) used in fertility treatments. In women, it is indicated for the stimulation of multifollicular development in patients undergoing superovulation as part of assisted reproductive technologies (ART), such as in vitro fertilization (IVF). It is also used for the induction of ovulation and pregnancy in anovulatory or oligo-ovulatory women who have not responded to clomiphene citrate. In men, follitropin alfa is used to stimulate spermatogenesis in those with hypogonadotropic hypogonadism, in conjunction with human chorionic gonadotropin (hCG).
How It Works
Follitropin alfa acts by binding to the follicle-stimulating hormone (FSH) receptor, which is primarily located on the granulosa cells of the ovarian follicle in women and the Sertoli cells of the testes in men. In women, this binding stimulates follicular growth and maturation. In men, it stimulates spermatogenesis, particularly when administered concurrently with human chorionic gonadotropin (hCG) to ensure adequate Leydig cell stimulation and testosterone production.
Dosage & Administration
injectable
Adults: For ovulation induction in anovulatory women: An initial dose of 75 International Units (IU) daily for 7-14 days. The dose may be increased by 37.5 IU at weekly intervals, up to a maximum of 225 IU daily. Treatment continues until adequate follicular development is achieved, followed by hCG administration. For assisted reproductive technologies (ART): An initial dose of 150-225 IU daily for the first 5 days of treatment. Subsequent doses are individualized based on ovarian response, not exceeding 450 IU daily. Treatment continues until adequate follicular development is achieved, followed by hCG administration. For men with hypogonadotropic hypogonadism: 150 IU three times weekly, administered concurrently with hCG (e.g., 1000-2000 IU two to three times weekly). Treatment duration is typically at least 4 months, and may continue for 18 months or longer.
Children: Not recommended. Consult a physician for specific guidance.
Side Effects

- Injection site reactions (e.g., pain, bruising, swelling, irritation) (approximately 5-10%)
- Nausea (approximately 5-10%)
- Bloating (approximately 5-10%)
Serious Side Effects
- Ovarian Hyperstimulation Syndrome (OHSS), severe cases occurring in less than 1% of cycles
- Ectopic pregnancy (increased risk in ART patients)
- Multiple gestations (up to 30% in ART cycles)
- Thromboembolic events (rare)
- Ovarian torsion (rare)
- Pulmonary and vascular complications
Contraindications
Follitropin alfa is contraindicated in patients with primary ovarian failure, uncontrolled non-gonadal endocrinopathies (e.g., thyroid, adrenal, or pituitary disorders), sex hormone-dependent tumors of the reproductive tract and accessory organs, abnormal uterine bleeding of undetermined origin, ovarian cysts or enlargement not due to polycystic ovarian syndrome (PCOS), primary testicular failure, or known hypersensitivity to follitropin alfa or any component of the formulation.
Drug Interactions
No clinically significant drug-drug interactions have been definitively established for follitropin alfa. Concomitant use with clomiphene citrate may enhance the follicular response. No known food interactions.
Pregnancy & Lactation
Category: X
Follitropin alfa is contraindicated in pregnant women. It is used to achieve pregnancy and should not be administered once pregnancy is confirmed due to potential harm to the fetus.
Storage
Store follitropin alfa in the refrigerator at 2°C to 8°C (36°F to 46°F) in the original carton to protect from light. Do not freeze. Once reconstituted (if applicable for specific formulations), it should be used immediately. Some multi-dose formulations may allow storage at room temperature for a limited period (e.g., up to 28 days), but always refer to the specific product's instructions for precise storage guidelines after first use.
Clinical Tips
Patients should receive thorough instruction on proper injection technique, storage, and the critical importance of regular monitoring by their healthcare provider. This monitoring is essential to assess ovarian response, adjust dosing as needed, and minimize the risk of Ovarian Hyperstimulation Syndrome (OHSS) and multiple gestations. Patients should be advised to report any severe abdominal pain, significant bloating, or sudden weight gain immediately to their doctor or pharmacist.
Frequently Asked Questions
What is follitropin alfa used for?
Follitropin alfa is a medication used in fertility treatments. It helps stimulate the development of eggs in women undergoing assisted reproductive technologies or those with ovulation problems, and it stimulates sperm production in men with certain hormonal deficiencies.
How is follitropin alfa administered?
Follitropin alfa is administered as an injection, typically subcutaneously (under the skin). Your doctor or nurse will provide detailed instructions on the correct preparation and injection technique.
What is Ovarian Hyperstimulation Syndrome (OHSS) and what should I do if I suspect it?
OHSS is a serious complication where the ovaries become excessively swollen and painful. Symptoms may include severe abdominal pain, significant bloating, nausea, vomiting, and sudden weight gain. If you experience these symptoms, seek immediate medical attention.
Does follitropin alfa increase the chance of multiple pregnancies?
Yes, treatment with follitropin alfa, especially in assisted reproductive technologies, increases the risk of multiple gestations (e.g., twins, triplets). Your healthcare provider will monitor you closely to manage this risk.
How should I store follitropin alfa?
Follitropin alfa should be stored in the refrigerator between 2°C and 8°C (36°F to 46°F) and protected from light. Do not freeze. Always refer to the specific product's instructions for detailed storage guidelines, especially after the first use of a multi-dose pen.
