Uses & Indications
Semaglutide tablets are indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus. It is also indicated to reduce the risk of major adverse cardiovascular events (cardiovascular death, non-fatal myocardial infarction, or non-fatal stroke) in adults with type 2 diabetes mellitus and established cardiovascular disease.
How It Works
Semaglutide is a glucagon-like peptide-1 (GLP-1) receptor agonist. It works by mimicking the effects of natural GLP-1, a hormone that plays a role in blood sugar regulation. Semaglutide increases glucose-dependent insulin secretion from pancreatic beta cells, decreases glucagon secretion from pancreatic alpha cells, slows gastric emptying, and promotes satiety, leading to reduced food intake and weight loss. These actions collectively contribute to improved glycemic control.
Dosage & Administration
tablet
Adults: The initial dose is 3 mg orally once daily for 1 month. After 1 month, the dose should be increased to 7 mg once daily. If additional glycemic control is needed after at least 1 month on 7 mg once daily, the dose may be increased to 14 mg once daily. The maximum recommended daily dose is 14 mg. Tablets should be taken at least 30 minutes before the first food, beverage, or other oral medications of the day with a sip of water (no more than 120 mL).
Children: Semaglutide tablets are not recommended for use in pediatric patients.
Side Effects
- Nausea (very common, affecting up to 20-30% of patients, especially during dose escalation)
- Diarrhea (common, affecting 10-20% of patients)
- Constipation (common, affecting 5-15% of patients)
- Vomiting (common, affecting 5-15% of patients)
- Abdominal pain (common, affecting 5-10% of patients)
- Decreased appetite (common)
Serious Side Effects
- Pancreatitis (inflammation of the pancreas, characterized by severe abdominal pain)
- Gallbladder disease (cholelithiasis or cholecystitis)
- Severe hypoglycemia (low blood sugar), especially when used with insulin or sulfonylureas
- Acute kidney injury or worsening of chronic renal failure
- Serious allergic reactions (e.g., anaphylaxis, angioedema)
- Diabetic retinopathy complications (in patients with a history of retinopathy)
Contraindications
Semaglutide is contraindicated in patients with a personal or family history of medullary thyroid carcinoma (MTC) or in patients with Multiple Endocrine Neoplasia syndrome type 2 (MEN 2). It is also contraindicated in patients with a known hypersensitivity to semaglutide or any of its excipients.
Drug Interactions
Semaglutide can delay gastric emptying, potentially affecting the absorption of concomitantly administered oral medications. Patients taking oral medications with a narrow therapeutic index should be monitored. When co-administered with insulin secretagogues (e.g., sulfonylureas) or insulin, the risk of hypoglycemia is increased, and a dose reduction of the concomitant medication may be necessary. Monitor thyroid function when co-administering with levothyroxine.
Pregnancy & Lactation
Category: Not formally assigned by FDA, but generally considered C/D based on animal studies.
There are limited data on semaglutide use in pregnant women. Animal studies have shown reproductive toxicity. Semaglutide should be discontinued at least 2 months before a planned pregnancy due to its long half-life. Consult your doctor or pharmacist if you are pregnant, planning to become pregnant, or breastfeeding.
Storage
Store semaglutide tablets at room temperature, between 15°C and 30°C (59°F and 86°F). Keep the tablets in their original blister packaging to protect them from moisture and light. Do not remove the tablet from the blister until immediately before use.
Clinical Tips
Take semaglutide on an empty stomach with a small amount of plain water (no more than 120 mL). Do not crush, cut, or chew the tablet; it must be swallowed whole. If you experience persistent severe abdominal pain, with or without vomiting, seek immediate medical attention as this could be a symptom of pancreatitis. Consistency in taking the medication at the same time each day is important for optimal effectiveness.
Frequently Asked Questions
How should I take semaglutide tablets?
Take semaglutide tablets once daily on an empty stomach, at least 30 minutes before your first food, beverage, or other oral medications of the day. Swallow the tablet whole with a sip of plain water (no more than 120 mL). Do not crush, cut, or chew the tablet.
What should I do if I miss a dose of semaglutide?
If you miss a dose, take it as soon as you remember, provided it is within 12 hours of your regularly scheduled dose. If more than 12 hours have passed, skip the missed dose and take your next dose at the regularly scheduled time. Do not take two doses to make up for a missed one.
Can semaglutide help with weight loss?
Yes, weight loss is a common side effect of semaglutide. It works by slowing gastric emptying and promoting a feeling of fullness, which can lead to reduced food intake and subsequent weight reduction. However, it is not specifically approved as a weight-loss drug in this formulation.
What are the most common side effects of semaglutide tablets?
The most common side effects include nausea, diarrhea, constipation, vomiting, and abdominal pain. These side effects are often mild to moderate and tend to decrease over time as your body adjusts to the medication, especially during dose escalation.
How long does it take for semaglutide to start working?
Semaglutide starts to affect blood sugar levels relatively quickly, but its full glucose-lowering and weight management effects are typically observed over several weeks to months as the dose is gradually increased and the body adjusts. Consistent use as prescribed is key for optimal results.
