Uses & Indications

Etoricoxib is a non-steroidal anti-inflammatory drug (NSAID) that selectively inhibits cyclooxygenase-2 (COX-2). It is indicated for the symptomatic relief of osteoarthritis, rheumatoid arthritis, ankylosing spondylitis, and the treatment of acute pain and primary dysmenorrhea. It is also used for the treatment of acute gouty arthritis and chronic low back pain.

How It Works

Etoricoxib works by selectively inhibiting the cyclooxygenase-2 (COX-2) enzyme. COX-2 is primarily responsible for the synthesis of prostaglandins, which are mediators of pain, inflammation, and fever. By inhibiting COX-2, etoricoxib reduces the production of these prostaglandins, thereby alleviating pain and inflammation. Its selectivity for COX-2 over COX-1 is intended to reduce gastrointestinal side effects associated with non-selective NSAIDs.

Contraindications

Etoricoxib is contraindicated in patients with known hypersensitivity to etoricoxib or any component of the formulation, active peptic ulceration or gastrointestinal bleeding, severe hepatic dysfunction, severe renal disease (creatinine clearance < 30 mL/min), inflammatory bowel disease, congestive heart failure (NYHA II-IV), established ischemic heart disease, peripheral arterial disease, and/or cerebrovascular disease. It is also contraindicated in patients who have experienced asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs.

Drug Interactions

Concomitant use with warfarin may increase the risk of bleeding; close monitoring of INR is advised. Co-administration with ACE inhibitors or angiotensin receptor blockers (ARBs) may reduce their antihypertensive effect and increase the risk of renal impairment. Diuretics may have their natriuretic effect reduced. Lithium levels may be increased when taken with etoricoxib, requiring monitoring. Methotrexate toxicity may be increased. Oral contraceptives may have increased estrogen exposure. Rifampicin may decrease etoricoxib plasma levels.

Pregnancy & Lactation

Category: C (first and second trimesters), D (third trimester)

Etoricoxib should be avoided during pregnancy, especially in the third trimester, due to potential risks of premature closure of the fetal ductus arteriosus and renal dysfunction in the fetus. Use in the first and second trimesters should only be considered if the potential benefit outweighs the potential risk to the fetus. Consult your doctor before use during pregnancy.

Store below 30°C. Keep in the original package to protect from moisture. Keep out of reach of children.

Clinical Tips

Etoricoxib can be taken with or without food. For acute pain conditions, use the lowest effective dose for the shortest possible duration. Patients with pre-existing cardiovascular risk factors should be monitored closely. Do not exceed the maximum recommended daily dose. If you experience any unusual symptoms or side effects, consult your doctor or pharmacist immediately.
